False PHD is a passive item introduced in The Binding of Isaac: Repentance. Found primarily in the Devil Room and Curse Room item pools, it provides a high-risk, high-reward twist on the standard pill mechanics. Pill Conversion: Converts all “good” stat-modifying pills into their “bad” counterparts (e.g., Tears Up becomes Tears Down).
